Filling out the AgComm Form 1 and Form 1A

All AgComm publishing projects begin with a completed ACS Form 1 ("Approval for Agricultural Communication Production"). This page explains the parts of the Form 1.

The form was updated in 2012.

Before we can begin your project we must have a completed form 1 that includes, at the very least:

  • The signature of your department head or Extension coordinator
  • Account numbers to be charged for production and account numbers for income
  • The signature of your business office representative (department comptroller)
  • If it is an Extension publication: names of two peer reviewers AND the signature of your Extension Program Leader.

Below is a detailed description of how to fill out the fields on the form.

Part I [Completed by author(s)/department]

Part 1 of Form 1

Title

Write your project's tentative title here. You may change the publication's title before it is produced.

Date Initiated

Indicate the date you filled out the Form 1.

Author(s)/Client(s)

List all the author(s) here, including graduate students.

Dept.

List the department(s) of the author(s) initiating this project.

Date Needed

Indicate the date you want the finished product available. Remember, this should be a realistic date and agreed upon in consultation with an AgComm staff member. Please do not use ASAP.

Quantity

Indicate the number of copies you want printed or reproduced. This number should be a result of consultation andinput from AgComm.

For digital publications, leave this space blank.

Format/Status/Old Stock

Fill these sections out in consultation with your editor.

Dept. Head/Dept. Extension Coordinator (signature-REQUIRED)

The authors' department head(s) or designee (such as the Purdue Extension coordinator) must sign and date here, indicating that the project is approved by the department for production. The signature indicates his or her approval of the publication's content and purpose.

 

Part II (filled out ONLY for Extension projects)

Part 2 of Form 1

Peer Reviewed by: (2 required)

Every numbered Purdue Extension publication requires at least two peer reviewers. Their names should be listed here.

Program Leader (signature)

Every numbered Purdue Extension publication requires a signature of the appropriate Extension Program Leader. The author's Extension program leader must sign and date here if this project is for an Extension-funded item. If the Program Leader is also the Department Head and/or Assistant Dean, that individual will have signed in Part 1 above and isn't required to repeat that approval here. The program leader's signature indicates his or her approval of publication's content and purpose.

 

Part III (account information/signature for spending)

Part 3 of Form 1

Fund for production costs

Indicate here the account number(s) to be used for printing and related costs.

If more than one department's funds are to be used, please include each account number and the percentage of costs to come out of each account.

If your project is a standard Web-only publication, account numbers are not required.

Remember: AgComm can only use Purdue University accounts.

Fund for Income

Indicate here the account number(s) where proceeds from the sale of educational materials should be deposited (if different from the production account).

If more than one department's funds are to receive income, please include each account number and the percentage of income that should be distributed to each account.

If your project is a standard Web-only publication, account numbers are not required.

Department Comptroller (signature-REQUIRED)

The department comptroller must sign and date where indicated before AgComm can begin the production process. This authorizes AgComm to use funds from the accounts listed to cover production and delivery costs.

If your project is a standard Web-only publication, account numbers are not required.

Retail Price

If this item will be sold, indicate the retail price here. That price and other items in this section should be determined in consultation with an AgComm staff member.
