Hoosier Agribusiness Science Academy (HASA) Summer Institute

The Office of Multicultural Programs (OMP) in the College of Agriculture at Purdue University will host 2, one day regional programs for middle school students and a two-week residential pre-college summer enrichment program for high school students.

Application Checklist
(Applications will not be considered unless all the following materials are completed and submitted.)
 
  1. Student Application Form
  2. Guidance Counselor Recommendation Form
  3. Teacher/Community Member Recommendation Form
  4. Official School Transcript – provided by your school  (Minimum 2.75 G.P.A.)

  

Applications can be submitted:

  • Online (preferred): Student Application; Guidance Counselor Form; Recommendation Form; transcripts
  • Email: Scan completed application forms and email to: omp@purdue.edu 
  • Fax transcripts and completed application to: Office of Multicultural Programs - College of Ag - 765-494-7420
  • Postal Mail to: Office of Multicultural Programs 615 W. State St. Room #106, West Lafayette, IN 47907

 

All forms must be submitted by May 15th 2014
