AUDIT REPORT FORMS:

Wholesale permits require reporting of egg distribution based upon the number of cases (30 dozen eggs per dozen) per week. These forms are sent to wholesalers in the last month of the covered period by the Indiana State Egg Board.  All reports are due on the date listed below.  Reports that are received past the due date will be charged a late fee.

Bi-Annual Reporting: Required for wholesalers distributing fewer than 5 cases of eggs per week and for Farmers Market Retail/Wholesale combination permit holders. Report periods cover:

  • January 1 – June 30 – Due by July 31
  • July 1 – December 31 – Due by January 31

Quarterly Reporting: Required for all wholesalers distributing 5 or more cases of eggs per week. Report periods cover:

  • January 1 – March 31 – Due by April 30
  • April 1 – June 30 – Due by July 31
  • July 1 – September 30 – Due by October 31
  • October 1 – December 31 – Due by January 31
